Segmental edema of the sigmoid and rectal mucosa with diffuse hyperemia, intramucosal hemorrhages, and flat ulcers. Luminal narrowing at the rectosigmoid junction due to edema. Crohn’s disease. SES‑CD: 4 points.

Histological conclusion
Focal mucosal lesions with lymphocytic and neutrophilic infiltration and marked macrophage infiltration of the lamina propria. Slit‑like ulcer parallel to the surface; epithelioid granulomas present. Findings consistent with Crohn’s disease.
